The following information sheet is designed to assist law enforcement and emergency personnel to respond to emergency situations which may involve your child or relative with autism, dementia or special needs.
This information sheet will provide pertinent information which may lessen anxiety and fear for the individual and help law enforcement/ emergency personnel better respond to the situation.
This form was developed for an individual with autism, but can also be filled out for those with Alzheimer’s and other needs in which this information would benefit.
